This service is supported by Commonwealth Home Support Program (CHSP) funding from the Australian Government Department of Health, as well as Home and Community Care Program for Younger Persons   (HACC PYP) funding from the Victorian Department of Health & Human Services (DHHS)

Social Support Group program is HACC PYP and CHSP funded providing person centred care and social support to enhance independence through a range of activities, community connections, outings, therapies and exercise.  Programs are designed around member strengths, interests and capabilities with a focus on nutrition, physical activity, emotional wellbeing and social skills.  Each member is empowered through Active Service Model to learn new skills and maintain their current level of functioning through setting personal goals which are reviewed annually.  Priority is given to those members of the community who have no or few social supports, respite for carers and those living on their own.

 Morning and afternoon tea, and a two course midday meal is included in the cost of the day.  Transport to and from the Centre is available within the Hamilton City boundary.

 Monthly “Out and About” Group – respite for carers day is available.  This is normally held on the third Saturday of each month – contact office for further information.

Referral

Referrals may be made by GPs, Health Professionals, Community Aged Care Package Managers, Home and Community Care Assessment team, family, friends and self-referral.

All referrals go on the waiting list. Initial contact is made by phone to acknowledge the referral, explain the service and a flyer is sent by mail.  The preferred / available days clients wish to attend are discussed and noted.

 Referrals are made to other services to assist with maintaining health and wellbeing.
