Resi-In-Reach
The Residential-In-Reach service includes:
Provision of clinical support for staff in Residential Care Facilities and to deliver timely clinical assessment and treatment in the residential facility (in collaboration with the resident’s treating GP).
The aim of the service is to provide an alternative to a transfer of the resident to the Emergency Department or coordinate a fast-track approach to care in hospital.
Referrals
- By phone enabling immediate support and advice, followed by prompt attendance to the resident at the facility.
- Residents may be referred by GPs and nursing staff.
Eligibility
All residents of Aged Care Facilities in Hamilton, Penshurst and Coleraine.
Cost
Service is provided by WDHS free of charge. Residential-In-Reach service comes to the facility.
