It’s about helping you live the best life you can. We offer several residential aged care services including respite, the Transition Care Program, and permanent placement.

Our person-centred care is founded on meeting your needs while respecting your choices to encourage independence and build self-confidence, whatever the level of care and support you choose and need. You’ll receive a higher standard of tailored care from qualified nurses 24-hours a day. You’ll also have quicker, easier access to local acute and allied health services. Our placements and services

Permanent placement

You can access our high-quality permanent residential care across all our aged care properties. There are many reasons why you may wish to move into permanent accommodation. We customise your care to ensure your preferences are met and we are considerate of your choices.

Our Aged Care Placement Coordinator can: make an appointment for you to view your preferred property and meet our dedicated staff; arrange a pre-admission assessment with the independent Aged Care Assessment Service, a multi-disciplinary team of health professionals; take you through the Resident Agreement document, and answer your initial financial questions.

Respite

Respite care is available across all our aged care properties when you’re intending to remain in your home and are seeking short-stay accommodation while your carer is absent. As a short-stay resident, you’ll have access to the full range of social activities and day outings, as well as full use of all facilities and amenities

Our Aged Care Placement Coordinator can: make an appointment for you to view your preferred property and meet our dedicated staff; arrange a pre-admission assessment with the independent Aged Care Assessment Service, a multi-disciplinary team of health professionals; and book your short stay. A fortnight before your arrival you’ll meet with the accommodation’s Unit Manager for a pre-admission interview.

Transition Care Program

Our Transition Care Program (TCP) offers up to 12-weeks support if you’re aged 65+, are recovering after a hospital admission, and living within a 40km radius of Hamilton. A referral begins during your hospital admission so your Transition Care Program can commence following your hospital discharge.

Each program is individually designed with your input and consent to increase your independence when returning home and into the community. Our TCP Nurse Case Manager will visit you weekly to assess your progress, care needs, and coordinate services with our multidisciplinary team.

How Aged Care finance works

Your aged care fees are as individual as you. Depending on your Department of Health means-tested assessment, some of these costs may be met in part or in full by the Australian Government. Fees and charges are adjusted bi-annually in line with the pension as prescribed by the Aged Care Act.

If you’re living as a permanent aged care resident, you’ll receive a monthly WDHS account covering your accommodation and day-to-day living costs. Like your fellow residents, you’re encouraged to open a Patient Deposit Trust Account with the Aged Care Administration Assistant at your aged care home to assist with managing your living expenses such as outings, haircuts, special activities, shopping, newspapers, and chemist or medication bills.

Our Aged Care Placement Coordinator can answer your initial financial questions. They can also speak with you about the Resident Agreement document if you’re considering moving into permanent aged care. This document outlines the care and services you can expect to be provided and optional services which incur separate fees.
