The Home Care Packages (HCPs) Program provides care in the home for those residents who are able to, and wish to stay in their own homes, with the support of personal care services. Packages are flexible to respond to the needs of individuals and may include services including:
- Personal care
- Social support
- Transport to appointments
- Home help
- Meal preparation, and
When you are accepted by the service your worker will discuss the options available to you and an agreement will be developed. Your carer, family members or representatives will be able to participate in negotiations of the types and levels of care to be provided. You may be asked to pay a fee for the service being provided.
An assessment by the Aged Care Assessment Service (ACAS) must be completed before you can be accepted into the service.
